This policy applies to clients (“clients”), complementary alternative medicine providers (“providers”), and visitors/guests who are interested in using our website at connect4healing.com (our “Website”).

This policy describes the types of information we may collect from you or that you may provide when you visit our Website and our practices for collecting, using, maintaining, protecting, and disclosing that information. The term “you” may apply to the client or the provider, or together, the client and provider.

We are not a treatment facility but only the platform that connects the client and provider. It will be the provider’s responsibility to provide, if required under HIPAA, a patient consent form to their client for any medical or other treatment they may provide.

This policy applies to the information we collect:

• On this Website.

• On our platform.

• By email, text, and other electronic messages between you and this Website.

• From you (the provider) and the communications you have with clients who are interested in your services.

• From communications between the client and provider on the Website (i.e., videoconferencing sessions).

• Through desktop applications that you download from this Website, which provide dedicated non-browser-based interaction between you and this Website.

It does not apply to information collected by:

• Us about our Website visitors, other than information that visitors freely provide.

• Us offline or through any other means, including on any other website operated by Connecting4Healing or any third party (i.e., your payment information); or

• Any third party, including through any application or content (i.e., a service provider that processes your payment) that may link to or be accessible from or on the Website.

Children Under the Age of 18

Information We Collect About You and How We Collect It

We collect several types of information from and about the clients and providers of our Website, including information:

• By which you may be personally identified, such as name, title, business contact information, postal address, e-mail address, telephone number, IP address, device ID, or any other identifier by which you may be contacted online or offline ("personal information");

• That the client may provide such as the state of residence, date of birth, gender, and account credentials such as your username, password, and payment information (i.e., credit card information).

• That the provider may provide relating to provider’s credentials, such as the state licensed to practice, date of birth, service type, availability (i.e., calendar), type of license, diploma, certificates, drivers’ license, professional references, past experience (i.e., resume), gender, bank account information, and social media information (i.e., Instagram, Facebook, and Twitter).

• That the provider and client exchange during a videoconferencing session (or voice or messaging) on our platform that may be Protected Health Information ("PHI") and/or medical information under the Health Insurance Portability and Accountability Act (“HIPAA”). For example, if the client and provider exchange PHI during video conferencing, the provider is responsible for providing to the client a patient consent form.

• That is about you but individually does not identify you, such as de-identified or anonymized data; and/or

• About your internet connection, the equipment you use to access our Website, and usage details.

How we collect this information:

• Directly from you when you provide it to us. For example, we may collect PHI from you if you participate in videoconferencing with a covered entity.

• From either client or provider that is PHI under HIPAA. If provider is a “Covered Entity” as defined under HIPAA, provider is responsible for providing us a Business Associate Agreement (“BAA”) to comply with HIPAA. For your convenience, we have a copy of a BAA we can supply upon request.

• Automatically as you navigate through the Website. Information is collected automatically may include usage details, IP addresses, and information collected through cookies and other tracking technologies.

• From third parties, for example, our business partners (such as analytics and email marketing service providers.

Information You Provide to Us.

The information we collect on or through our Website may include:

• Information that you provide by filling in forms on our Website. This includes information provided at the time of registering to use our Website, subscribing to our service, posting material, or requesting further services. We may also ask you for information when you report a problem with our Website.

Stripe
To process payments, we use Stripe, a third-party payment processor. Stripe may collect and process personal information, including billing details, payment card information, and other relevant data in order to complete the transaction.

Healthie
In order to provide certain services, we use third-party platforms, such as Healthie, to manage aspects of our telehealth services. Healthie is a third-party software provider that may collect and store personal health information on behalf of your healthcare provider's ("Provider").

By using our services, you agree to the collection, processing, and storage of your personal and health data by Healthie in accordance with their Privacy Policy. Healthie’s privacy policy outlines how they handle your personal information, including their data protection practices, retention periods, and your rights under applicable data protection laws. 

Disclosure of Your Information

We may disclose or use aggregated or de-identified information about you or information that does not identify any individual, without restriction.

We may disclose personal information that we collect or you provide as described in this privacy policy:

• To our subsidiaries and affiliates.

Choices About How We Use and Disclose Your Information
